Please use the online application process if at all possible.

If it is impossible for you to use the online application process, follow these instructions instead:

• Download and print an Application Form from here.
• Download and print the Learner Initial Task. This is available in a choice of two formats:
  • Microsoft Word document
  • Adobe Acrobat
• Complete the Application Form and the Initial Task in full
• Return them together by either
  • post to Family Matters Institute, Moggerhanger Park, Moggerhanger, Beds. MK44 3RW
  • fax to Family Matters Institute on 01767 641515
• The remainder of the process is the same as for the online application process
